    摘要:

    目的 探究低血清间接胆红素水平与脑卒中溶栓后脑出血转化的相关性分析。方法 选取2016年12月—2019年10月在该院进行静脉溶栓治疗的175例急性缺血性脑卒中患者。分为脑出血转化组12例和非脑出血转化组163例;另选取同期于200例该院体检并检查结果健康者作为对照组。检测所有研究对象溶栓前总胆红素、直接胆红素、间接胆红素水平。多因素Logistic回归分析影响缺血性脑卒中溶栓后脑出血转化发生的因素。结果 与对照组相比,缺血性脑卒组患者血清总胆红素、间接胆红素水平较高(P<0.05)。与非脑出血转化组比较,脑出血转化组患者心房纤颤发生率、大面积脑梗死发生率、溶栓前舒张压、NIHSS评分较高(P<0.05)。脑出血转化组患者溶栓前血清总胆红素、间接胆红素水平低于非脑出血转化组(P<0.05)。回归分析显示,高NIHSS评分、低间接胆红素是缺血性脑卒中溶栓后脑出血转化的危险因素。结论 低血清间接胆红素水平与缺血性脑卒中溶栓后预后呈负相关。低间接胆红素是缺血性脑卒中溶栓后脑出血转化的危险因素。

    Abstract:

    Objective To investigate the association of low serum level of indirect bilirubin with cerebral hemorrhage transformation after thrombolysis in patients with stroke.Methods A total of 175 patients with acute ischemic stroke who underwent intravenous thrombolysis in our hospital from December 2016 to October 2019 were enrolled and divided into cerebral hemorrhage transformation group with 12 patients and non-cerebral hemorrhage transformation group with 163 patients; 200 individuals who underwent physical examination during the same period of time and had healthy results were enrolled as control group. The levels of total bilirubin, direct bilirubin, and indirect bilirubin were measured for all subjects before thrombolysis. A multivariate logistic regression analysis was used to investigate the influencing factors for the development of cerebral hemorrhage transformation after thrombolysis in patients with ischemic stroke.Results The ischemic stroke group had significantly higher serum levels of total bilirubin and indirect bilirubin than the control group (P<0.05). Compared with the non-cerebral hemorrhage transformation group, the cerebral hemorrhage transformation group had significantly higher incidence rates of atrial fibrillation and massive cerebral infarction, diastolic pressure before thrombolysis, and National Institutes of Health Stroke Scale (NIHSS) score (P<0.05). Before thrombolysis, the cerebral hemorrhage transformation group had significantly lower serum levels of total bilirubin and indirect bilirubin than the non-cerebral hemorrhage transformation group (P<0.05). The multivariate logistic regression analysis showed that high NIHSS score and low indirect bilirubin were risk factors for cerebral hemorrhage transformation after thrombolysis in patients with ischemic stroke.Conclusions Low serum indirect bilirubin is negatively correlated with the prognosis of patients with ischemic stroke after thrombolysis, and low indirect bilirubin is a risk factor for cerebral hemorrhage transformation after thrombolysis in patients with ischemic stroke.
