颅内动脉粥样硬化狭窄相关急性大血管闭塞早期血管内治疗的研究进展

    摘要:

    早期血管内治疗已成为大血管闭塞性缺血性脑卒中的标准再灌注治疗方法。而颅内动脉粥样硬化狭窄是亚洲人大血管闭塞性缺血性脑卒中的主要病因之一。对于此类病变,进行急诊机械取栓时容易发生早期再闭塞,相比较心源性栓塞常需要更多的补救措施、更长的手术时间,以实现血管再通。其最佳治疗策略尚没有大规模循证医学证据支持。因此术前尽早预判动脉粥样硬化性狭窄病变、制定合理、高效的血管内治疗策略至关重要。本文就近年来ICAS相关LVO的血管内治疗研究作一总结。

    Abstract:

    Early endovascular therapy has become the standard reperfusion therapy for large vascular occlusive(LVO) ischemic stroke. Intracranial atherosclerotic stenosis(ICAS) is one of the main causes of vascular occlusive ischemic stroke in Asian people. However,nearly 30% of stroke patients could not receive reperfusion,which may lead to poor functional outcomes. This type of LVO is often associated with reocclusion in endovascular thrombectomy procedures. Compared with cardiogenic embolism, it often requires more rescue measures and longer operation time to achieve succesful recanalization. There is no clear consensus on the optimal endovascular treatment strategy for patients with ICAS-LVO.Therefore, it is very important to early diagonose ICAS-related LVO and formulate the appropriate endovascular treatment strategy before operation. This review summarizes the recent researches on endovascular treatment of ICAS-related LVO.
