    摘要:

    目的:评估运用3D Slicer软件辅助功能在神经内镜经鼻蝶切除鞍区肿瘤手术的意义和价值。 方法 回顾性分析本院自2017年7月-2021年7月收治的60例鞍区肿瘤手术治疗患者的临床资料,根据是否应用3D Slicer软件辅助功能分为观察组(应用3D软件辅助,n=30)和对照组(单纯神经内镜经鼻蝶入路手术,n=30)。比较两组的肿瘤切除程度、手术时间、术中出血量,比较激素水平改善情况和术后并发症等。 结果 全部患者均安全完成手术,术后无一例发生严重合并症。观察组在术中出血量、手术时长、住院时间方面均优于对照组,其中术中出血量、手术时长优势明显(P<0.05),在降低海绵间窦损伤、脑脊液漏的发生更有优势(P<0.05)。术后的内分泌紊乱、视力损害的缓解率,观察组要优于对照组,并且优势显著(P<0.05)结论 3D Slicer软件辅助神经内镜经鼻蝶入路切除鞍区肿瘤,做到高精度术前规划,有利于缩短手术时间,手术安全性较高,有助于降低术后并发症发生率,具有较为良好的实用价值和临床意义。

    Abstract:

    Objective To evaluate the significance and value of 3D slicer software in endoscopic transnasal transsphenoidal resection of sellar tumors. Methods A total of 60 patients with sellar region tumors who underwent endonasal transsphenoidal surgery in our hospital from July 2017 to July 2021 were analyzed retrospectively. According to whether 3D slicer software was used for auxiliary function, they were divided into observation group (assisted by 3D software, n = 30) and control group (simple endoscopic transnasal transsphenoidal approach, n = 30). The extent of tumor resection, operation time, intraoperative bleeding, hormone level improvement and postoperative complications were compared between the two groups. Results All patients underwent the operation safely, and no serious complications occurred after operation. The observation group was better than the control group in the amount of intraoperative bleeding, operation time and hospital stay (P < 0.05). The remission rate of endocrine disorder and visual impairment after operation in the observation group is better than that in the control group, and has significant advantages (P < 0.05). Conclusion 3D slicer software assisted endoscopic transnasal transsphenoidal approach for resection of sellar tumors can achieve high-precision preoperative planning, which is conducive to shortening the operation time, high operation safety and reducing the incidence of postoperative complications, It has good practical value and clinical significance.
