80例带状疱疹患者炎症因子、免疫球蛋白、T淋巴细胞亚群与后遗神经痛的关系

    摘要:

    目的 探讨带状疱疹患者炎症因子、免疫球蛋白、T淋巴细胞亚群与后遗神经痛的关系。方法 纳入2020年10月至2022年10月在汨罗市人民医院神经内科就诊的带状疱疹患者(80例)。根据患者有无合并后遗神经痛分成合并组(32例)和未合并组(48例)。对比两组一般资料、疼痛介质(血浆P物质、β-内啡肽、降钙素基因相关肽、神经降压素、神经生长因子诱导蛋白)、炎症因子(C反应蛋白、白细胞介素-6、肿瘤坏死因子-α、降钙素原)、免疫球蛋白(免疫球蛋白A、免疫球蛋白G、免疫球蛋白M)、补体C3、补体C4、T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)水平。采用皮尔逊相关系数分析炎症因子、免疫球蛋白、T淋巴细胞亚群与后遗神经痛的关系。结果 两组患者一般资料、肿瘤坏死因子-α、降钙素原和T淋巴细胞亚群水平比较差异无统计学意义(P>0.05)。合并组血浆P物质、降钙素基因相关肽、神经生长因子诱导蛋白、C反应蛋白、白细胞介素-6高于未合并组(P<0.05);β-内啡肽、神经降压素、免疫球蛋白A、免疫球蛋白G、免疫球蛋白M、补体C3、补体C4低于未合并组(P<0.05)。皮尔逊相关系数分析显示,C反应蛋白、白细胞介素-6与血浆P物质、降钙素基因相关肽、神经生长因子诱导蛋白水平呈正相关(P<0.05);与β-内啡肽、神经降压素水平呈负相关(P<0.05)。免疫球蛋白A、免疫球蛋白G、免疫球蛋白M、补体C3、补体C4与血浆P物质、降钙素基因相关肽、神经生长因子诱导蛋白呈负相关(P<0.05);与β-内啡肽、神经降压素呈正相关(P<0.05)。结论 带状疱疹患者易合并后遗神经痛,与炎症因子过度表达和免疫功能低下相关。

    Abstract:

    Objective To investigate the association of inflammatory factors, immunoglobulins, and T lymphocyte subsets with postherpetic neuralgia in patients with herpes zoster.Methods A total of 80 patients with herpes zoster who attended Department of Neurology, Miluo Peoples Hospital, between October 2020 and October 2022 were enrolled, and according to the presence or absence of postherpetic neuralgia, they were divided into comorbidity group with 32 patients and non-comorbidity group with 48 patients. The two groups were compared in terms of general data, pain mediators (plasma P substance, β-endorphin, calcitonin gene-related peptide, neurotensin, and nerve growth factor induction protein), inflammatory factors (C-reactive protein, interleukin-6, tumor necrosis factor-α, and procalcitonin), immunoglobulins (immunoglobulin A, immunoglobulin G, and immunoglobulin M), complement C3, complement C4, T lymphocyte subsets (CD3+, CD4+, CD8+, and CD4+/CD8+). The Pearson correlation coefficient was used to identify the correlation of inflammatory factors, immunoglobulins, and T lymphocyte subsets with postherpetic neuralgia.Results There were no significant differences between the two groups in general data and the levels of tumor necrosis factor-α, procalcitonin, and T lymphocytes (P>0.05). Compared with the non-comorbidity group, the comorbidity group had significantly higher levels of plasma substance P, calcitonin gene-related peptide, nerve growth factor induction protein, C-reactive protein, and interleukin-6 (P<0.05) and significantly lower levels of β-endorphin, neurotensin, immunoglobulin A, immunoglobulin G, immunoglobulin M, complement C3, and complement C4 (P<0.05). The Pearson correlation analysis showed that C-reactive protein and interleukin-6 were positively correlated with plasma substance P, calcitonin gene-related peptide, and nerve growth factor induction protein (P<0.05) and were negatively correlated with β-endorphin and neurotensin (P <0.05). Immunoglobulin A, immunoglobulin G, immunoglobulin M, complement C3, and complement C4 were negatively correlated with plasma substance P, calcitonin gene-related peptide, and nerve growth factor induction protein (P<0.05) and were positively correlated with β-endorphin and neurotensin (P<0.05).Conclusions Patients with herpes zoster often have the comorbidity of postherpetic neuralgia, which is associated with the overexpression of inflammatory factors and low immune function.
